Bellewood & Brooklawn is the only provider in Kentucky that utilizes the Psycho-Educational Model (PEM), an evidence-based approach that has proven highly effective with youth in our care. Our services include in-home support, community-based, therapeutic foster care, independent living programs, and ongoing training for youth, parents, and social workers. This broad range of services allows us to remain a trusted partner throughout all phases of care, from initial placement to ongoing support.
Admissions Information
Bellewood & Brooklawn welcomes referrals for state-committed youth and those who remain in the care of their biological families. We work closely with families and the Department of Community-Based Services (DCBS) to ensure that each child is placed in the most appropriate setting for their needs.

Required Documentation for Residential Referrals
To make a residential referral, please provide the following documents:
- Psychiatric Evaluation
- Psychosocial Assessment
- History & Physical
- IEP or 504 Plan
- Progress/Shift Notes
- List of Current Medications
- List of All Previous Placements and Outpatient Providers
- Demographics/Face Sheet
On the day of admission, please ensure the following items are provided:
- Birth Certificate
- Social Security Card
- Immunization Records
- Insurance Card(s)
- Guardianship Papers
- Two Weeks of Clothing
- School Uniform (Jean/Khakis; solid color polo or button-down with collar)
