Our services include in-home support, community-based, therapeutic foster care, independent living programs, and ongoing training for youth, parents, and social workers. This broad range of services allows us to remain a trusted partner throughout all phases of care, from initial placement to ongoing support.

Admissions Information

Bellewood & Brooklawn welcomes referrals for state-committed youth and those who remain in the care of their biological families. We work closely with families and the Department of Community-Based Services (DCBS) to ensure that each child is placed in the most appropriate setting for their needs.

Required Documentation for Residential Referrals

To make a residential referral, please provide the following documents:

  • Psychiatric Evaluation
  • Psychosocial Assessment
  • History & Physical
  • IEP or 504 Plan
  • Progress/Shift Notes
  • List of Current Medications
  • List of All Previous Placements and Outpatient Providers
  • Demographics/Face Sheet

On the day of admission, please ensure the following items are provided:

  • Birth Certificate
  • Social Security Card
  • Immunization Records
  • Insurance Card(s)
  • Guardianship Papers
  • Two Weeks of Clothing
  • School Uniform (Jean/Khakis; solid color polo or button-down with collar)
